Thelma M. Blankenship, 83, of Medford, died Saturday (Jan. 1, 2000) at Farmington Square in Medford. No service is planned. Interment was in Hillcrest Memorial Park, Medford. Memorial contributions may be made to Providence Community Health Foundation, Hospice, 1111 Crater Lake Ave., Medford, OR 97504, or the American Cancer Society, P.O. Box 698, Medford, Or 97501. She was born Thelma M. Embrey on Oct. 4, 1916, in Waurika, Okla. On Aug. 1, 1935, she married Alva A. Blankenship, who died in 1977.
She moved to the Rogue Valley in 1945 from California. She moved to the Portland area in 1959; to Mesa, Ariz., in 1976; and returned to Medford in 1995. Survivors include a son, Donald W., Central Point; a daughter, Barbara A. Daniel, Livermore, Calif.; five grandchildren; 11 great-grandchildren; and one great-great-grandchild. Don was a respected leader in health care,holding hospital administrator positions at Watonga Municipal Oklahoma, Osteopathic Hospital in Tulsa, University Heights Hospital in Albuquerque, N.M.,Enid Memorial Hospital and Jefferson County Hospital at Waurika, Oklahoma.
He is survived by his wife Myrtle, a daughter Deanna of Alton, Ill. , two sons Cliff of Albuquerque, N.M. and Larry of Nowata, Okla.

PHELPS celebrate his life and announce his joyous reunion with his Lord on Sun., July 9, 2006. - Don, 74, a longtime resident of Waurika, passed away in Jefferson County Hospital of multiple myeloma. A celebration service will be held at 10 a.m. Tues. in First Baptist Church in Waurika. Arrangements are under direction of Dudley Funeral Home of Waurika. - Don was born Nov. 9, 1931, to Joseph Eugene "Gene" and Minnie Phelps in Waurika. He married Myrtle I. Proctor on Feb. 14, 1951, in San Antonio, Texas, while serving in the U.S. Army at Fort Sam Houston. - Don was a respected leader in health care, holding hospital administrator positions at Watonga Municipal Hospital, Oklahoma Osteopathic Hospital in Tulsa, University Heights Hospital in Albuquerque, N.M., Enid Memorial Hospital and Jefferson County Hospital. - Don received the W. Cleveland Rodgers Distinguished Service award in 1991, the highest honor that can be given to an Oklahoma health care executive. He served on various boards and delegations in connection with his profession. After his retirement from his executive position at Jefferson County Hospital, he continued to serve others in the health insurance field up until his illness in 2006. - Don served his Lord as deacon in First Baptist Church in Watonga, Ranch Acres Baptist Church in Tulsa, University Heights Baptist Church in Albuquerque, Emanuel Baptist Church in Enid and First Baptist Church in Waurika. - He also served in civic leadership positions on the Waurika City Council and many other service organizations. - Survivors include his wife, Myrtle of Waurika; a daughter, Deanna Phelps and son-in-law, Stephen Hager of Alton, Ill.; two sons, Cliff Phelps and daughter-in-law, Johnelle of Albuquerque, and Larry Phelps of Nowata; a brother, Alton Phelps and wife Peggy of Duncan; a sister, Evelyn Knecht of Sweetwater, Texas; six grandchildren, Adam Davis and wife Tamra, Emily Davis, Alexandra and Jordyn Phelps, Justin Phelps and Josh Salisbury; and three great-grandchildren. - His parents, Gene and Minnie Phelps, preceded Don's arrival in glory. - Jean Added: Sep. 20, 2007
